The Centers for Medicare & Medicaid Services’ (CMS) Rural Health Transformation Program (RHTP) represents one of the most significant federal healthcare investments in recent history, providing $50 billion in funding over five years to support state-led initiatives that improve access, quality, workforce sustainability, technology adoption, and long-term health outcomes in rural communities.

While much attention has been placed on developing Rural Health Transformation Plans and securing funding approval, the award itself is only the beginning. The long-term success of RHTP initiatives depends on a state’s ability to demonstrate continued progress, maintain compliance with program requirements, and effectively manage the governance and reporting obligations that accompany federal funding.

CMS has established clear expectations for ongoing accountability. States are required to report regularly on program implementation, work plans, timelines, milestones, and measurable outcomes. These reporting requirements are intended to ensure that funding is used in accordance with approved transformation strategies and that investments produce meaningful results for rural communities.

As implementation begins, many states face a common challenge: transitioning from planning and funding acquisition to sustained program execution. Success requires more than project management. It demands a structured approach to compliance, performance monitoring, documentation, stakeholder coordination, and program governance.

Extending or aligning RHTP oversight with these existing governance models, rather than standing up a parallel structure, reduces administrative burden while strengthening the state’s overall enterprise view.

At the same time, CMS continues to place greater emphasis on Medicaid program integrity, transparency, and accountability for federal investments. States that proactively establish compliance monitoring and governance frameworks will be better positioned to demonstrate program success and protect funding throughout the initiative’s life.
